Do I need an appointment, or do you accept walk-ins?

All appointments are scheduled — we do not accept walk-ins. To be seen, call your nearest center or send a request through this site and we will arrange a time.

What insurance plans do you accept?

We accept a range of insurance plans. Because coverage varies, we recommend contacting your nearest center before your first visit so we can check your specific plan and explain your benefits.

Do you offer lab services?

Yes — we partner with Intervene One Lab for laboratory services.

How do I schedule an appointment?

Call your nearest center directly, or complete the Contact Form through this site and a member of our team will contact you to arrange a time.

What should I bring to my first appointment?

A valid photo ID, your insurance card, a list of your current medications including doses, and any recent lab work or records from other providers. If you use a glucose meter or CGM, bring it or your recent readings.

What payment options do you accept?

We accept cash, credit and debit cards, and most major insurance plans. If you have questions about cost or payment arrangements, contact your nearest center.

What should I do in a diabetes emergency?

For any emergency, call 911.
